Vision Vouchers

CVIM is able to offer vision vouchers that cover a basic eye exam, basic lenses, and basic frames. To qualify for the voucher you must have no vision insurance, meet financial guidelines, and be a resident of Pennsylvania.

You do not have to be a CVIM patient to obtain a vision voucher.

 The vision vouchers can be used at Nittany Eye Associates, Heimer Eye Care Associates, Central Penn Vision, Restore Eye Care, and State College Eye Care. We recommend that diabetic individuals who obtain a voucher use them at Nittany Eye Associates, Restore Eye Care, or State College Eye Care.

 The recipient of the voucher is financially responsible for anything not covered by the voucher, including, but not limited to: upgrades to frames, lenses, coatings, contact lens exams, and contact lenses.

The Pennsylvania Vision Foundation, an affiliate of VBA, is a 501(c)(3) organization committed to preserving vision health through optometry in Pennsylvania. Since 2007, it has helped over 10,382 uninsured Pennsylvanians access quality eye care.

Hearing Aid Program

The purchase of hearing aids for low-income individuals is often impossible due to the expense of the device.  The hearing aid program at CVIM breaks down this barrier by providing financial assistance in conjunction with available hearing aid programs and/or assistance with the purchase. To qualify for this program, you must be a Centre County Resident and meet income guidelines.

You do not have to be a CVIM patient to use the Hearing Aid Program.

 Our Case Manager will set up an appointment for you at the State College Sam’s Club to obtain the hearing aid unit(s). You will not receive a bill for the unit(s). This program is funded by generous grants and donations.

Breast Health

CVIM is able to offer breast health vouchers that cover mammograms, ultrasounds, biopsies, and other early detection testing. To qualify for the voucher you must have no insurance or insurance that does not cover testing, meet financial guidelines, and live or work in Centre County.

You do not have to be a CVIM patient to obtain a Breast Health Voucher.

 The vouchers can be used at Mount Nittany Breast Care Center or Geisinger Breast Care in Gray’s Woods
